The Vitalize 360 at Kendal-Crosslands Communities combines an innovative, person-directed approach to wellness coaching with the power of information derived from a scientifically-grounded assessment system.  The program engages, challenges, and inspires our residents to live full, healthy, vibrant lives and enables our progressive retirement community to demonstrate significant improvement in successful aging.

A Commitment to Improve Healthy Aging

At Kendal~Crosslands Communities, we know the interests, preferences, wellness and challenges of our residents. And, we’re committed to improving their vitality.

Vitalize 360 increases the quality and depth of engagement between staff and residents. We  target the right programs and resources to support improved well-being and successful aging.

Vitalize 360 is a joint venture between two not-for-profits: Hebrew SeniorLife, a Harvard Medical School Affiliate, and Kendal.  Kendal~Crosslands has also collaborated with Vitalize 360 and offers this program under the direction of our Wellness Coach.

Reflective, Person-centered Coaching

Vitalize 360 focuses on “what matters most,” not on “what’s the matter.” Residents meet with our Vitalize 360 coach to set goals, share what’s most important to them, and practice focusing on living more intentionally.  The coach supports residents as they progress through stages of activation towards living their best lives.

Creating a Vitality Plan

Residents work with their staff coach to create a personalized Vitality Plan. It’s a well-being roadmap that guides the achievement of self-identified goals, supports existing activities, and inspire new achievements. 

The plan starts with two evidence-based assessments. First, the self-administered Lifestyle Survey captures the resident’s preferences and satisfaction with current activities.  Then, the Health and Social Check-up covers health, community and social life, cognition and mental health and getting around in everyday life.
